Requesting appointments at Caring Hearts Animal Hospital is easy! Simply fill out the fields below and click “Submit.” A member of our staff will then contact you to confirm your pet’s appointment. Your appointment is not booked until we contact you.

Please note: This form is intended for non-emergency visits only. If your pet has an urgent health concern and needs immediate veterinary care, please call us at (704) 893-2799 during our normal business hours. For after-hours emergencies, please call an emergency veterinary hospital.
